There is a particular kind of invisibility that doesn't announce itself. It doesn't feel like exclusion — it just feels like the norm, until someone points out that the norm was built around someone else entirely.
For decades, health and fitness research operated on a quiet assumption: that the female body is essentially a smaller version of the male one, complicated occasionally by hormonal fluctuation, but fundamentally following the same rules. Adjust the dosage. Scale the numbers down. The framework stays.
New research is challenging that assumption at its foundation. A growing body of work — including a significant analysis published this year — argues that women have been systematically underrepresented in health and fitness studies not just in terms of numbers, but in terms of design. Studies weren't simply missing women. They were designed in ways that made women's data inconvenient: the hormonal variation across a menstrual cycle, the shifts through perimenopause and menopause, the physiological responses to training and recovery that diverge meaningfully from male patterns. The easier choice, for generations of researchers, was to study men and extrapolate. The assumption was that the extrapolation was good enough.
It wasn't. The consequences are practical and they are sitting inside every gym, every GP's consultation, every fitness app that tells you to eat more protein and train harder without once asking where you are in your cycle.
I think about this every time I walk into a gym in Valletta and see the same generic programming on the wall — the same rep counts, the same rest periods, the same nutritional targets — offered equally to a 22-year-old man and a 45-year-old woman in perimenopause, as if the same key opens both locks. Fashion, at its worst, does this too: designs a silhouette for one body and calls it universal. Health research has been doing the same thing with data.
What the newer science suggests is not that women need softer versions of exercise or protection from intensity — quite the opposite. It suggests that women's bodies respond to training, nutrition, and recovery in ways that are distinct enough to require their own evidence base. Strength training appears to be particularly beneficial across hormonal life stages. Recovery windows may differ. The assumption that less is always more for women in the weight room has no solid backing.
The good news is that this conversation is finally happening loudly enough to change research design. The less comfortable news is that the guidelines most of us have been following were built on a foundation that assumed we were someone else.
One thing to do: If you train regularly, spend fifteen minutes this week looking at whether your programme was designed with female physiology in mind — or simply adapted from a male template. The difference matters more than the marketing suggests.
